Does every change need a form? No. Routine reorders of the same listing and pack (class 0) go on the batch record with a gate check. Pack-size changes need a short class 1 form; wick, jar, dye and process changes class 2; wax and oil changes class 3.

Why write the retest plan before testing? So the plan cannot bend to fit the result. Box 4 is signed before the first test candle is poured.

A change-control form is one page that stops a change being made without anyone deciding to make it.

Small candle businesses rarely lack test results. What they lack is the link between a change and its evidence: a note of what changed, why, for which candles, what was retested and who said yes. Without that link, a year later nobody can tell whether a candle's wax was changed deliberately or drifted there, and the next person to touch the formula has no way to know what they are undoing. The form below is designed to fit on one printed page, be filled in by hand, and take ten minutes for a small change. Copy the table as it stands.

Three fields do most of the work. From / To forces the exact listing, which is where most uncontrolled changes hide — the specification post explains why a name is not enough. Affected SKUs catches the change that was meant for one candle and quietly reached six. And the retest matrix in box 4 is filled in before the tests are run, so the plan cannot be adjusted to fit the result. Everything else is bookkeeping, and bookkeeping is what lets the next person trust the first three.

A retest plan written after the results is a description of what happened, not a plan. The form's value is that it commits you to the evidence before you know what it will say.

Four change classes, so small changes stay small

A form that demands the same effort for a new bag size as for a new wax will be skipped. So the form begins with a class, and the class decides which boxes are needed. The classes use the series vocabulary: a change is classed by the worst retest status it triggers in its matrix. The example counts in the last column are for an illustrative 12-SKU business over a year; they show the shape most businesses see — many small changes, a few large ones.

The class is a rule of thumb for effort, not a substitute for the matrix. A jar change is usually class 2, but a jar that measures differently at the base can raise end-of-life questions that push it towards class 3 — the jar-only post shows which lines repeat. Similarly a dye change is usually class 2, but the revised-formulation post sets out when jars, dyes, additives or process changes are big enough to treat the candle as a new formula. Class 0 still gets written down
A routine reorder does not need a form, but it does need its lot label and gate check on the batch record — the reproducibility record has the fields. Class 0 is only safe because that record exists.

Worked example 1: a class 1 change that turns out to cost money

The smallest changes are the ones most likely to be made without a form, and the ones most likely to hide a surprise.

Purchasing wants to move a SKU's Luxury Soy Wax Chunks from 5 kg bags to 10 kg bags “to get a better price”. On the form, box 1 records the same listing and handle with a different pack size; the class is 1. Box 2 is where the surprise appears. On CSI's 24 September 2026 listed prices the 5 kg bag is ₹2,995.00 (₹599.00 a kilo) and the 10 kg is ₹5,999.00 (₹599.90 a kilo). The bigger bag is 90 paise a kilo dearer, which is ₹0.17 more per 200 g candle. The stated reason is wrong, and the form catches it before the order is placed.

“It's the same wax, it doesn't need a form.” The material may well be the same. The reason given for the change was not true, the cost moved in the wrong direction, and the costing sheet would have been wrong for a year. A class 1 form takes five minutes and finds exactly this kind of mistake.

Worked example 2: a class 3 wax change on four SKUs

The second change is real: moving four SKUs from Luxury Soy Wax Chunks to Premium Coconut Soy Wax CSI 468. The reason is the reader's own (say, a range repositioned as coconut-soy); the cost moves by ₹9.38 per candle at 1 kg listed prices. It is class 3, because a wax change voids every test line and CSI 468 has no fragrance ceiling published in this data, which adds a NEW line. Safety acceptance on the form is always comparative. CSI publishes no numeric flame-height or jar-temperature limit, so box 5 can only record “no worse than control” on each safety line, within your jar supplier's guidance and the recognised candle fire-safety standard you work to (ASTM F2417 is a commonly cited one). A retained control candle has aged; note its batch date on the sheet.
